Our care planning process involves a detailed examination of their medical records and any related documents, conducting an interview with the client to gain historical information, and when feasible from family members. Discussions with the treating health care providers are conducted and life expectancy is considered. Cost data is obtained based on the geographical location of the client. This process allows us to develop a customized care plan that often includes:

  • Future Therapeutic Modalities: Which may include necessary physical therapy, occupational therapy, cognitive therapies and other rehabilitation services to aid recovery/maintain level of function.
  • Future Routine Medical Treatment: Identification of necessary medical treatments including doctor’s visits, radiographic imaging, injection therapy, procedures and/or surgeries. 
  • Home Furnishing and Equipment: Identify durable medical equipment or other items in home, workplace to facilitate and maintain level of functioning.
