A life care plan is also known as a “long term care plan.” It is a document that can aid in decision-making by individuals with disabilities or chronic illnesses who are faced with a variety of options for treatment or service delivery.
A life care plan can ensure that the injured, disabled, or chronically-ill person has an adequate strategy that addresses future care throughout their life expectancy. A life care plan can be updated or modified to reflect changes in the patient’s condition or circumstances. The plan is typically developed in collaboration with the patient’s physician(s), nurse practitioner(s), physical therapist(s), and other healthcare professionals involved in caring for the patient.
A life care planner will work with you to develop a plan tailored to your specific needs and goals.
